DOCUMENTS / INFORMATION

We would require the following to be forwarded by post or fax to the Export Department at Wockhardt UK Ltd

1. A prescription from your doctor listing the exact product and quantities required.

2. An undertaking from your doctor, that he or she will report any serious adverse reactions to the products within 48 hours and any non-adverse reactions within 10 days to Wockhardt UK Ltd and also to the appropriate authorities in your country.

3. The full delivery address (please note - this must be for a Doctor or Pharmacy).

PAYMENT

Upon receipt of the above we would then ask for payment in advance of shipment using one of the following methods.

1. Credit Card (Visa, Mastercard, Access or Eurocard)
An order form will be sent for you to complete and return to the Export Department at WockhardtUK Ltd. Payment will then be obtained using the information provided.

2. Swift or Telegraphic Transfer
A pro-forma invoice will be sent to you outlining the payment required. You will then need to arrange for that amount to be transferred to our bank account which will be detailed on the pro-forma invoice.

3. Cheque
A pro-forma invoice will be sent to you outlining the payment required. You will then need to send a cheque for the full value to the address below:

DELIVERY

Upon receipt of payment we will arrange for your order to be packed and despatched by TNT courier. You will be advised of the TNT Airwaybill Number, which you can use to monitor the package during transit, using the tracking option available at www.tnt.co.uk. We will also follow the progress of your order.
